Jazzar – DDS Surgeon-Aide: Chadi Maroun Anesthetist-Reanimation : Dr. Moussa Abboud Anesthetist-Reanimation Aide: Mrs. Chadia Nurse: Mrs. Joyceline Photograph/Video: Alain Daoud – Studio Daoud Meziara] Dental Implant System: ITI® - Straumann Group – Switzerland Implant ME Distributor: Prodent S.A.L - Beirut Dental Technician: Pegase – Sarkis Kelaghbian Procedure: SURGERY STEP I-Before Surgery: Diagnostics Tests: • NUMEROUS TYPES OF X-RAYS AND OTHER TESTS SUCH AS BLOOD TESTS WILL PROVIDE INFORMATION TO EVALUATE TREATMENT PLAN. • PANOGRAPHIC X-RAY......PROVIDES A VIEW OF THE ENTIRE MOUTH, LOCATION OF SINUSES, NERVES AND INDICATES ANY BONE PROBLEMS. • TOMOGRAPHY X-RAYS (CT SCAN).....PROVIDES CROSS- SECTIONAL AND OTHER VIEWS THAT MEASURE YOUR PRECISE JAW HEIGHT, WIDTH AND BONE DENSITY. • BLOOD TESTS......AN ONGOING MEDICAL CONDITION SUCH AS DIABETES, WILL DETERMINE IF YOUR CONDITION IS UNDER CONTROL. [DONE AT AL-KOURA HOSPITAL BEFORE THE SURGERY DAY]. Special Surgical Procedure: • BONE GRAFT......BONE GRAFTING WILL INCREASE THE HEIGHT AND WIDTH OF THE JAW. AFTER THE NEW BONE IS PACKED AROUND THE`JAW, THE GUM IS CLOSED [SUTURES] AND THE NEW BONE BEGINS FUSING WITH YOUR JAW. NATURAL MINERAL BONE [BIO-OSS 0.5 ] IS USED. II-Implant Surgery: Extraction 13 teeth: 46, 45, 44, 43, 42, 41, 31, 32, 33, 34, 35, 36, 37 Implants: The Standard Plus Implant is used because it is the best flexible solution for trans-, semi- or subgingival implant placement in the esthetic region. Three prosthetic diameters were used depending of the location: • Narrow Neck - NN: Implants with the 3.5 mm Narrow Neck (NN) restorative platform (Standard Plus Implant with a built-in octa abutment with external connection, for single-tooth restorations). • Regular Neck - RN: Implants with the 4.8 mm Regular Neck (RN), restorative platform. • Wide Neck - WN: Implants with the 6.5 mm Wide Neck (WN), restorative platform. Implants site: 10 SLA ITI Solid Screw Implants [SLA= Sand-blasted, Large grit, Acid-etched surface] 41: @3.3 SP lg 10mm NN 43: @4.1 SP lg 14mm RN 44: @4.1 SP lg 14mm RN 46: @4.8 SP lg 12mm WN 47: @4.8 SP lg 10mm WN 31: @3.3 SP lg 12mm NN 33: @4.1 SP lg 14mm RN 34: @4.1 SP lg 14mm RN 36: @4.8 SP lg 12mm WN 37: @4.8 SP lg 10mm WN Closure screws on all implants small or large or for NN and the following: 34: RN healing cap h: 2.0mm Ti 36: WN healing cap h: 2.0mm Ti 37: WN healing cap h: 2.0mm Ti 46: WN healing cap h: 2.0mm Ti [RN= Regular Neck, WN= Wide neck, NN= Narrow Neck, TI=Titanium] Bone Grafts Bio-Oss® granules 0.5 g – Natural bone mineral (Implantation material) Sutures III- After Surgery: Follow up: Post surgical treatments were given by perfusion under hospital control for 48 hours. Augmentin 1.2: by I.V direct x3/24 hours Profenid 100: Ampoule by I.V x3/24 hours Zantac: Ampoule I.V direct x3/24 hours Perfalgan 1gr.: by I.V direct – every 6 hours. Chemical plaque control with chlorexidine-digluconate (0.12%) was recommended in the first 3 weeks following the first 24 hours of implants placement. Ice Meals: Jello, Custer, milk, puree, soup, soft cheese, soft meat, cold tea… Panoramic XRay one week after surgery. Patient’s conditions & complications: Bleeding: None Infection: None Nerve damage: None. Swelling: None Muscle Injury: None Healing: Very good HEALING & OSSEOINTEGRATION STEP Check osseointegration after 15 days. Implant loaded between 15 days after implant surgery. PROSTHETIC STEP (Lower Full Fixed Denture) -2 Crowns of white gold/ceramic on 47 and 37. -2 Bridges of 3 white gold/ceramic crowns each on 46-44 and 34-36. -1 Bridge of 6 white gold/ceramic crowns on 43-33. Upper full mouth rehabilitation was implemented on December 2006. This Immediate implantation case will be given as a lecture during the "17th International Dental Journeys of Nice" in France on 7th of June 2007.
